In the current age of medicine it’s always good to consider that a person can be medically well and functionally sick. Our passion is to take your proactive referral and create a positive outcome beyond expectations.

FOX Geriatric House Calls™ allow us fully to assess the patient, the home environment, and the caregiver’s circumstances. Our clinicians then provide clinically excellent care that is custom-tailored for each patient, guided by evidence-based practices and standards.

Medical doctors, nurse practitioners, registered nurses, social workers, and geriatric care managers have been referring patients to FOX since 1998. Undoubtedly our delivery of consistently positive outcomes is why.

Primary Care, General Practitioners, Geriatricians

Older adults do not have to be hospitalized, have a nursing need, or be homebound to access specialized rehab solutions in the home setting. FOX’s therapeutic approach is individualized to each patient and aggressively applies current evidence to optimize function, including activities of daily living, and quality of life while reducing risk of hospitalization, falls and improving safety (Falvey, 2016; Sherrington,2011; Leland, Elliot, & Johnson, 2012; Siebert, Smallfied, & Stark, 2014).

Neurologists

Older adults living with progressive neurological disorders, for example Parkinson’s disease and dementia, require proactive and evidence-based therapeutic interventions throughout their lives to maximize their functional wellness and function. Those with chronic conditions, including cerebrovascular accident (CVA), still benefit from the skilled application of motor learning principles, task-specific training, and compensatory strategies months to years after disease onset (National Parkinson Foundation’s (NPF) Parkinson’s Outcomes Project, 2016; Ciucci, 2013; Hopper, 2005; Blankewoort, 2010 ; Hardage, 2011 ; Allen, 2012 ; Pimouguet, 2017; Preissner, 2014).

Pulmonologists

Older adults living with respiratory disorders require skilled clinicians that thoroughly understand the physiology to optimally treat each patient’s unique limitations as part of the healthcare team. FOX clinicians utilize current evidence to treat your patients while monitoring their physiological responses to ensure proper referral and follow up as needed (Bolton, 2013; McKeogh, 2012; Klijn, 2013; O’Shea, 2009; Ramos, 2014; Gosselink, 2011).

Cardiologists

Many of the most impactful chronic diseases in older adults are cardiovascular in nature. Individuals living with cardiovascular disease are at risk of poor health outcomes, hospitalization, and less likely to age in place. FOX therapy is carefully dosed by clinicians who are adept in treating those with cardiovascular compromise. They continually monitor patient response, refer as necessary, coordinate care, and promote self-management and independence (Healthcare Cost and Utilization Project (HCUP) Statistical Brief Trends in Hospital Readmissions for Four High–Volume Conditions, 2009-2013).

Orthopedic Specialists

Chronic

Many older adults are living with pain or orthopedic limitations that severely impact their quality of life and limit their function. These individuals often resort to opioids or surgical intervention that may also negatively impact their ability to age in place. The CDC recently advised that physicians refer to physical therapy as a first step in managing chronic pain (Dowell, 2016).

Acute

More older adults are undergoing elective orthopedic procedures such as total knee and total hip arthroplasties (TKA/THA) as they are living longer. Today’s healthcare environment stresses quality outcomes that are cost effective. FOX’s innovative house calls model is an approach that does both. Patients post TKA that go directly to outpatient therapy, without home health services, demonstrate similar outcomes within a shorter amount of time (49.1 vs 68.1 days) and less overall visits and cost to the system (Warren, 2012).
